Diagnosis

Diffuse leptomeningeal metastasis of signet cell carcinoma of the colon. (ACR Case #419)

Note

Although uncommonly seen in children, adenocarcinoma is one of the most common causes of leptomeningeal tumor dissemination in adults.

Leptomeningeal metastasis in children is uncommon and is usually related to subarachnoid spread of primary cerebral neoplasms, especially medulloblastoma, ependymoma, and germinoma.  Highly malignant neoplasms such as glioblastoma multiforme may have leptomeningeal spread and even benign tumors such as pilocytic astrocytoma may uncommonly have subarachnoid dissemination.  Although cerebrospinal fluid (CSF) cytologic examination is still the "gold standard" for diagnosis of leptomeningeal carcinomatosis, contrast-enhanced MR has become a standard diagnostic tool in the detection of CSF tumor spread.

诊断：结肠的印戒细胞癌导致弥漫性软脑膜转移瘤。
注释：腺癌虽然少见于儿童，但仍是成人软脑膜转移的最常见原因。
小结：儿童的软脑膜转移瘤不常见，常由于脑原发肿瘤沿蛛网膜播散，如髓母细胞瘤、室管膜瘤、生殖细胞瘤。高度恶性的肿瘤比如，多形性胶质细胞瘤可发生软脑膜转移。甚至良性的毛细胞星形细胞瘤，偶尔也会蛛网膜下播散。虽然脑脊液细胞学检查是诊断软脑膜转移性癌的金标准，但增强MR也可作为脑脊液播散肿瘤的标准检查手段。
