有趣病例70：双肺散在小结节，什么病？

先看一张CT！

双肺散在小结节！

临床上经常遇到！

什么病？

转移性肺癌？不是很典型。典型的转移一般是大小不等的结节。当然了，如果有肿瘤基础，首先考虑转移。

感染？有可能！但是，什么类型的感染？

肺结核？病灶形态单一，不像，但是有可能。

血管炎？有可能，但是，不是很像。

尘肺？不像。结节太散！

类风湿结节？没有依据。

这种双肺散在小结节，诊断挺困难的！

其实，我也没有特别好的思路。只是今天偶然间看到这个病例，顺便和大家分享。

希望在病例学习中，慢慢的总结经验。

Objective: To study the CT appearances of primary pulmonary histoplasmosis (PPHP) and evaluate the diagnostic values of CT for PPHP.

Methods: The clinical materials and multi-slice spiral CT appearances in 8 patients with PPHP proved pathologically were reviewed retrospectively.

Results: Single or multiple pneumonia-like lesions were seen in 3 cases, single or multiple nodules or masses were in 4 cases, and both pneumonia-like lesions and nodules or masses in 1 case. Calcification and halo sign were common, and only slight enhancement was demonstrated after intravenous contrast administration on CT.

Conclusion: CT is helpful imaging method in the diagnosis and differential diagnosis of PPHP combining with clinical materials although the CT appearances are not specific.

目的：研究原发性肺组织胞浆菌病（PPHP）的CT表现，评价CT对PPHP的诊断价值。

方法：回顾性分析8例经手术病理证实的PPHP患者的临床资料和多层螺旋CT表现。

结果：单发或多发肺炎样病变3例，单发或多发结节或肿块4例，肺炎样病变合并结节或肿块1例。钙化和晕征是常见的，静脉注射造影剂后CT仅显示轻微强化。

结论：虽然CT表现不具特异性，但结合临床资料，CT对PPHP的诊断和鉴别诊断有一定的帮助。
